A “Safer” Habit With Hidden Risks

E-cigarettes have quickly become one of the most common alternatives to smoking, especially among younger men. Often marketed as a cleaner, less harmful option than traditional tobacco, vaping has been embraced by many men who believe they are avoiding the most serious health risks associated with smoking. As of 2023, as many as 6.5% of adults used e-cigarettes, and men were more likely to use them than women. (1)

However, while vaping may reduce exposure to some carcinogens, it raises a different, and often overlooked, concern: its impact on men’s sexual health.

Erectile dysfunction (ED) affects an estimated between 30 and 50 million men in the United States alone. (2) ED can undermine confidence, strain relationships, and even signal more serious health problems like heart disease. (3) Many men know that smoking is a major contributor to ED, but far fewer realize that vaping could carry many of the same risks.

All of this leads to the obvious question: can vaping contribute to ED? The answer, based on emerging research, is that it can play a role in enhancing erectile dysfunction in men. One study found that men who use e-cigarettes daily are more than twice as likely to report erectile dysfunction, even after controlling for cardiovascular factors (4)

While vaping is seen by some as a middle ground between smoking and quitting, the nicotine and chemicals in e-cigarettes can harm blood vessels, disrupt hormone balance, and impair the very systems that make erections possible.

In this guide, we’ll further explain the connection between e-cigarettes and ED, and what exactly research is saying about the connection between the two.

What Are E-cigarettes and How Do They Work?

At their core, e-cigarettes are devices designed to deliver nicotine without burning tobacco. A small battery heats an e-liquid until it becomes a vapor, which is then inhaled. That vapor usually contains nicotine, along with propylene glycol or vegetable glycerin as carriers, plus flavoring chemicals.

When it comes to health concerns, nicotine is the most worrisome ingredient.  Even though vaping avoids the tar and many toxins found in traditional smoke, nicotine still has powerful effects on the cardiovascular system. Even “nicotine-free” vape liquid can contain trace amounts of nicotine, sometimes exceeding legal limits. Additionally, heated flavorings and additives may irritate blood vessels and lungs, influencing long-term health (5)

Understanding Erectile Dysfunction

Erectile dysfunction is often more than just an issue in the bedroom. It is a condition that arises when any of the body’s major systems—vascular, hormonal, neurological, or psychological—fail to work together. Erections depend on increased blood flow to the penis, healthy nerve signaling, and sufficient testosterone levels. When something interrupts that chain, ED can develop.

The main causes of ED can be summarized as (6):

  • Vascular problems such as high blood pressure, atherosclerosis, and diabetes, which damage the blood vessels supplying the penis.
  • Hormonal imbalances like low testosterone or thyroid issues.
  • Neurological conditions that affect nerve signaling, including diabetes, multiple sclerosis, or spinal cord injuries.
  • Psychological factors such as stress, anxiety, or depression.
  • Other health related issues such as obesity, Parkinson’s disease, or MS.

The Link Between Nicotine and Erectile Function

Nicotine is a stimulant that delivers both immediate and long-term consequences for sexual health. In the short term, nicotine constricts blood vessels, making it harder for arteries in the penis to open and allow adequate blood flow. (7) Erections depend on that process of relaxation and expansion, so even temporary narrowing can weaken sexual performance.

Over time, nicotine exposure damages the endothelium, the inner lining of blood vessels, which can lead to stiffness and reduced flexibility. It also contributes to the buildup of fatty plaque inside arteries, a process known as atherosclerosis. (8) Combined with increased blood pressure and heart strain, these changes limit circulation throughout the body, including the pelvic region.

For men, this can often translate into weaker, less reliable erections. Even if men who smoke may not notice a problem right away, years of nicotine use can gradually erode their vascular health until ED becomes a regular part of their lives.

What the Research Says About Vaping and ED

For decades, the evidence linking smoking and ED has been overwhelming. But only recently have researchers begun focusing on e-cigarettes. While vaping reduces exposure to some toxins in cigarette smoke, it is not free from consequences.

Some studies have shown that daily e-cigarette users were more likely to report erectile dysfunction compared to non-users, regardless of age or cardiovascular risk. (9) Additional research has shown that vaping chemicals appear to increase oxidative stress, which damages cells and accelerates aging in blood vessels. They also contribute to low-grade inflammation, which further reduces circulation. (10)

In short, e-cigarettes may affect erectile function through several mechanisms:

  • Oxidative stress damaging penile blood vessels.
  • Chronic vascular inflammation reducing blood flow.
  • Hormonal disruptions affecting testosterone levels.
  • Nicotine’s vasoconstrictive effects limiting circulation during arousal.

Overall, current research shows that vaping may follow the same pathway as smoking: damage to blood vessels, reduced blood flow, and eventually, compromised erectile function.

Vaping vs. Smoking — Which Is Worse for Erectile Health?

When comparing vaping and smoking, it is important to recognize that cigarettes contain thousands of harmful chemicals, many of them directly toxic to blood vessels and reproductive health. By that measure, vaping is a less harmful alternative overall. However, less harmful is doesn’t mean safe.

While vaping may not be quite as destructive as smoking, it still undermines the foundation of healthy sexual function.

Can Quitting Vaping Improve ED?

The body has an impressive ability to heal when harmful exposures are removed. Men who stop smoking or vaping often see improvements in blood pressure, lung capacity, and circulation within weeks to months. Erectile function can also rebound, depending on how advanced the problem is, although it’s far from a guarantee. Research shows that around 25% of ex-smokers experience significantly better erectile function after a year. (11)

Still, quitting vaping gives the body the best possible chance to recover. While recovery timelines will vary for each patient, some men may notice improvement in sexual performance within a few months of quitting the use of e-cigarettes. (12) However, if ED is linked to permanent vascular damage, diabetes, or low testosterone, additional treatment may be necessary.

When to Seek Medical Advice for ED

Even for those who use e-cigarettes, not every instance of erectile dysfunction is necessarily a reason for concern. Certain factors, such as stress, fatigue, or alcohol use can all cause temporary difficulties with sexual performance. (13)  But persistent ED lasting more than three months warrants a professional evaluation, and it could be enhanced by routine nicotine use.

Consider seeking medical advice if you notice:

  • Difficulty achieving or maintaining erections consistently.
  • Low sex drive or persistent fatigue.
  • Mood changes or irritability.
  • Other health concerns such as high blood pressure or diabetes.
